HIPAA Privacy Notice

At Linda’s Helping Hands, LLC., we are committed to protecting the privacy and confidentiality of your health information. Federal law (HIPAA) requires us to safeguard your Protected Health Information (PHI) and to provide you with this notice describing how your information may be used, disclosed, and how you may access it.

Use and Disclosure of Your Health Information

We may use or disclose your PHI without written authorization for the following purposes:

Treatment:
To coordinate and provide your care, including sharing information with other providers involved in your treatment and contacting you with appointment reminders or health-related information.

Payment:
To bill and receive payment for services, including submitting claims to your insurance plan and verifying coverage.

Health Care Operations:
For activities such as quality assurance, staff training, auditing, accreditation, and general administrative functions.

We may also disclose PHI as required by law, including:

  • Reporting suspected abuse or neglect

  • Preventing or lessening a serious threat to health or safety

  • Health oversight activities

  • Judicial or administrative proceedings

  • Worker’s compensation matters

  • Public health and disaster relief efforts

When using or disclosing PHI, we follow the minimum necessary standard.

Uses Requiring Written Authorization

Any other use or disclosure of your PHI will require your written authorization, including:

  • Use of psychotherapy notes

  • Marketing communications

  • Sale of PHI

You may revoke any authorization at any time in writing.

Your Rights

Under HIPAA, you have the right to:

  • Request restrictions on how your PHI is used or disclosed

  • Request confidential communications by alternative means or locations

  • Inspect and obtain a copy of your health records (including electronic copies)

  • Request amendments to your records

  • Receive an accounting of certain disclosures

  • Be notified in the event of a breach involving your PHI

Requests must be submitted in writing. We will review and respond in accordance with applicable laws.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health & Human Services. We will not retaliate against you for filing a complaint.
